 Welcome back to RSC 2021/2022 Programs. 

 It is mandatory that we have certain documents signed and submitted on the first day that your child skates.  We have attached two forms that we need to have filled out and signed. 

1.  Skate Ontario Waiver (Junior, Senior skaters & Parent)- please sign and bring on first day.  This only has to be done once, however would be done for each Club if skating at more than one. 

2.  Rowan’s Law Acknowledgement Form for Athletes and Parents/Guardians (Senior Skaters only) Parents:  Please choose the appropriate booklet for your skater. The booklets are age oriented: 10 and under, 11-14 years and 15 and up.  https://skateontario.org/rowans-law-2/ 

Read through the booklet with your skater as well as the Skate Ontario Concussion Code of Conduct. Please print and sign the Rowan's Law-Athletes and Parents/Guardians Acknowledgement Form as attached.  

Skaters who are Star 5 and up (RSC Senior Skaters) and under 18 years of age must sign the form along with their parent/guardian. 

3.  RSC COVID-19 Health Screening Questionnaire - Complete the online Daily Covid-19 Screening form prior to your skater sign in time. Link as following or you may find it on the website (www.skateriverside.ca) :

Please hand 1) Skate Ontario Waiver, 2) Rowan's Law Acknowledgement Form for Athletes and Parents/Guardians  in to your coach at the arena on the first day of your training 

At the rink skaters and any person that is staying at the rink with them MUST sign the "Session Participation and Health Screening Tracking Form" upon arrival and complete online Daily COVID-19 screening form each session.
